Home Medicines Review (HMR)
Objectives:
- Improve quality use of medicines by comsumers living atr home through the HMR process
- Provide GPs with information on the HMR process
- Provide GPs with knowledge to access the HMR forms on their medical software
- Facilitate the HMR process for the GPs and pharmacists
Activities:
- Provide practice visits to GPs on the HMR process
- Provide CPD activities on HMR
- Provide training in clinical software to access HMR forms
- Provide phone/email helpdesk on HMR
- Provide hard copies of HMR forms when required by GPs
- Provide practice visits to pharmacies within the Division
