Joining the Division
South Eastern Sydney Division of General Practice Ltd (Division) offers two types of membership packages: Ordinary Membership and Associate Membership.
1. Ordinary Membership Application Form
According to the Division Constitution, an “Ordinary Member” means a Member of the Division who is a General Practitioner who practises for at least four (4) hours each week in a location which is within the area of the Designated Area.
2. Associate Membership Application Form
According to the Division Constitution, an “Associate Member” means a Member of the Division who is either not a General Practitioner or is a General Practitioner who practises in a location which is outside the area of the Designated Area.

Please complete the applicable form and return it with a cheque payable to The South Eastern Sydney Division of General Practice Ltd (SESDGP Ltd) for $66.00.
